چکیده
Material and Resources A descriptive, retrospective study. We describe the results of a small series of potential donors (including older than 70 years) treated in single tertiary level center, from January to September 2013, in accordance with a strict protocol of protective ventilation, recruitment maneuvers, postural changes and negative fluid balance. The collected variables were: Ventilatory parameters, central venous pressure, 24 hours fluid balance and information related to hemodynamic monitoring by transpulmonary thermodilution with Picco system (Pulse Contour Induced Cardiac Output).

A Protective Kidney-Lung Approach to Improve Outcomes in Mechanically Ventilated Patients.
There is increasing evidence that deleterious interactions between the lung and the kidney may be partly responsible for the multiorgan failure and high mortality seen in patients with acute respiratory distress syndrome. Lung protective strategies can reduce many of the adverse mechanistic and biological effects of mechanical ventilation. However, the key modifiable mediators are yet to be def...
متن کاملConventional mechanical ventilation is associated with bronchoalveolar lavage-induced activation of polymorphonuclear leukocytes: a possible mechanism to explain the systemic consequences of ventilator-induced lung injury in patients with ARDS.
BACKGROUND Protective ventilatory strategies have resulted in a decreased mortality rate in acute respiratory distress syndrome, but the underlying mechanisms remain unclear. The authors hypothesized that (1) mechanical ventilation modulates activation of polymorphonuclear leukocytes (PMNs), (2) the consequent release of proteinases is correlated with a systemic inflammatory response and with m...
متن کاملPositive Fluid Balance Is Associated with Higher Mortality and Prolonged Mechanical Ventilation in Pediatric Patients with Acute Lung Injury
Introduction. We analyzed a database of 320 pediatric patients with acute lung injury (ALI), to test the hypothesis that positive fluid balance is associated with worse clinical outcomes in children with ALI. Methods. This is a post-hoc analysis of previously collected data. Cumulative fluid balance was analyzed in ml per kilogram per day for the first 72 hours after ALI while in the PICU. The ...
متن کاملFluid balance and length of mechanical ventilation in children admitted to a single Pediatric Intensive Care Unit.
INTRODUCTION Associations between cumulative fluid balance and a prolonged duration of assisted mechanical ventilation have been described in adults. The aim of this study was to evaluate whether fluid balance in the first 48 hours of assisted mechanical ventilation initiation was associated with a prolonged duration of this process among children in the Pediatric Intensive Care Unit (PICU). ...
متن کاملComparison of two fluid-management strategies in acute lung injury.
BACKGROUND Optimal fluid management in patients with acute lung injury is unknown. Diuresis or fluid restriction may improve lung function but could jeopardize extrapulmonary-organ perfusion. METHODS In a randomized study, we compared a conservative and a liberal strategy of fluid management using explicit protocols applied for seven days in 1000 patients with acute lung injury. The primary e...
